But first, let us answer a few commonly asked questions: 

Days + Times:

Our program conveniently runs five afternoons and evenings a week, Monday through Friday, with start times between 2:00pm and 6:00pm. Clients must attend three to six consecutive hours a day (depending on their level of care) during which time they will experience multiple 60/90-minute psychotherapy and case management sessions with breaks for rest and meals in between.

Insurance Coverage:

Yes, we take most PPO insurance and handle all claims and billing concerns for you (leave the headache to us!) 25-50% refundable deposits may be required. We are currently negotiating HMO and MediCal contracts, and can attempt for a single case agreement which means we can ask your insurance company to consider us in-network for your treatment while we determine our partnership.

Levels of Care Available:

Partial Hospitalization (5 days a week, M-F, 6 hours a day, 3:00pm-9:00pm, virtual, 2-6 weeks)

Intensive Outpatient (3-5 days a week, M-F, 3 consecutive hours a day, between 3:00pm-9:00pm, virtual, 4-6 weeks)

When choosing the Type of Treatment you're seeking on the form below, Outpatient means the levels of care Exist offers, Inpatient + Outpatient means you think you'll need detoxification or 24/7 care prior to coming to our program and Inpatient only means you are wanting levels of care we do not offer.

Detox/Crisis/Emergencies: We do not provide these services. Please call 911 for life-threatening or active emergencies or 988 for mental health crisis and suicide help line.

In-Person Care: We are in the process of putting together our in-person Laguna Beach program. It should be open in mid-late 2024.

 

What happens after this form is submitted?

Our Team will be sent your information and we will contact you within 24 hours (once in operation) to complete a Pre-Admission Assessment and discuss Program Fees with you. If you submit health insurance information, we will attempt to contact your policy provider first so when we reach you we have as many details of your financial coverage as possible. Please be patient with us and if you have a time limitation, please note it at the end of the below form.

PLEASE NOTE: WE DO NOT BEGIN PROVIDING SERVICES UNTIL MID-APRIL 2023 AND WILL NOT RESPOND TO CLIENT APPLICATIONS UNTIL THEN.
